PVSA Form Submission Instructions

PLEASE READ THE PVSA POLICY HERE BEFORE SUBMITTING YOUR PVSA APPLICATION BY FOLLOWING THE OUTLINED STEPS BELOW

Must be an active 5C member and have participated in 5C activities within the past 12 months in order to  apply for PVSA through 5C.

  1. Click HERE to open 5C Leadership Volunteer Hour Tracking Template

  2. Select Download from the File menu to get the service log template in Microsoft Excel (.xlsx) format

  3. Fill in the service log on Excel

  4. Send an email to 5C Leadershipfoundation attaching the following documents:

  • Completed service log​

  • Proof of service time*

  • Birth Certificate, passport, or green card

* Note: NJHS/NHS form is NOT a proof of community service hours because anyone can sign the NHS hour sheets which are lack of validity. You must have the service hours validated by the organizations or institutes that you performed community service. For community services such as some food banks that have their own service forms with logo, these forms themselves are deemed the proof of community service. Screenshots of online service log with institute logo and valid approvals are acceptable. Emails or paper certificates that contain the service dates and hours are also valid service evidences. For 5C organized community services, you do not need to submit proof of service as the hours were recorded in the 5C master log.
